Fall (Nov/Dec) After School Programs Survey

To better plan fall after school programs with weather conditions and Covid-19 restrictions, we would like you to fill this survey. We will use the results to decide if we will continue offering vendor partnered after school programs and/or adjust program options in Nov/Dec. Please be aware that this survey is for interest collection purposes instead of program enrollment.

As some after school programs take place in school undercover areas, we need volunteers to help take lunch chairs down for afterschool programs and set them back up for lunch. We will release the volunteer opportunities once we finalize the plan.

The survey will be closed by Oct 11th, 2021. Thank you for your partnership.
